We never receive patient identifiers. Not at intake, not ever. The service is built around a de-identified intake protocol: you keep the name, MCP number, and date of birth on your side, and send only what the form's content actually requires — a patient label, demographics like age and sex, and the relevant dates and restrictions.

You re-identify the document on your end before printing and signing. The patient's identity never leaves your clinic, and you remain the author of every document you sign.

Do you ever see patient names or health-care numbers?

What about sick notes for patients still in the office?

This service is built for the forms that arrive after the visit — by fax or email — where same-day drafting matters most. Sick notes needed before a patient leaves are best completed in-clinic during the visit.

How do I pay?

You receive an invoice for $15 per form by email. Pay by credit card through the invoice (no account required) or by Interac e-transfer. That's it.
